Apple Community Health Services | ACHS, is a Georgia Department of Community Health and Georgia Medicaid approved Case Management firm that offers EDWP Case Management for SOURCE/CCSP eligible clients. We facilitates screening, assessing, planning, arranging, coordinating, delivery and evaluating service delivery for all applicants/clients in the Traditional/Enhanced EDWP.

Our mission is to build a trusted community of cross functional health care providers, health care advocates, technology experts and community members, committed to delivering personalized, client focused EDWP Case Management Services to all EDWP applicants/clients.
